PATIENT JOURNEY

Three Milestones:
Initial Appointment, Medical Titration and Remote Review
 

Triage begins collection of required blood tests, supportive diagnosis and pre clinic questionnaire number one initial appointment, treatment plan discussed and decided upon this is via zoom second appointment, six months on from the initial appointment, review of clinical progress and medication titration as required via zoom.

 

Subsequent appointments review all clinical goals and aim for achieving clinically stability on an established regime, at which point a patient is then eligible for remote review.

Appointment three, six months from second appointment, all goals, all clinical goals, achieved, clinically stable on an established regime, patient is then eligible for remote review. Typically, this is conducted on an annual basis. Please note, this is a typical patient journey, but this can vary depending on an individual's needs or health issues.

Number three, continuation of dose titration and review of clinical progress. Appointment four, this is typically 18 to 24 months of stable estrogen levels. Appointments concentrate on the discussion and consideration of adding in micronized progestogen.

 

Appointment five, six months review of the addition of progestogen if needed.  Once established on a stable regime of oestrogen and antiandrogen therapy, plus or minus progestogen, remote review is typically held on an annual basis. Please note, this is a typical patient journey, but this can vary depending on an individual's needs or health issues.

What is remote review?

Ongoing Clinical Remote Review

Remote review is conducted via completed medical form, along with review of blood tests, current medication, weight and blood pressure, full stop. This allows for safe prescribing and comprehended monitoring of patients who are taking gender affirming hormone treatments.

​Typically, this is conducted on an annual basis. Please note, this is a typical patient journey, but this can vary depending on an individual's needs or health issues. Occasionally, patients will need to be followed up by zoom, should it be clinically required. 

Shared Care With Your GP

We work hard to support your GP in being able to provide the hormone therapy we advise in your consultation. It is always best practice for patients, GPs and specialists to all work together from the outset.

Do you have a recent blood test with the required results?

Blood Test for Remote Review

Please arrange for test results at least one month before your remote review is due, to allow enough time for the results to be received. If your GP is unable to offer blood test, we recommend this service biomarkertracking.com/YGESV
